Entitlements Report

The Entitlements Report is designed to help entitlement provisioning agencies to verify the legitimacy of the application information supplied by individuals who try to enroll in the federal/state/local government aid programs such as food stamps, medical assistance, and social services.

By supplying comprehensive information on the applicants before enrollment, this report can help to identify potential unreported household composition and assets and therefore determine the eligibility of the application for assistance.

Order the Report

1. Click the Run a Report icon on the left of a record. You are prompted to choose a report. Entitlements Report is one of the options.



2. Click the Purchase Report button for the Entitlements Report. The Request Report window displays.



3. Click the Request Report button. In a few seconds the Entitlements Report populates the window.

Report Options

• Select Time Frame – The default is All Results, which returns subject records of all times. Or use the down arrow to select from Current Only; Most Recent 12 Months; Most Recent 24 Months; Most Recent 36 Months; or Most Recent 48 Months.
Picking any of the “Most Recent nn Months” will include those from “Current Only”.


• Select Report Type – The default is Interactive Web Page. It returns links for further research if necessary. (All you need to do is to click the desired link followed by selecting the proper search.) Or use the down arrow to select from Plain Text Web Page; Microsoft Word Document; or PDF Document.
  Caution: If you select Microsoft Word Document, you will need to download the report to a proper directory on your hard drive. It is recommended that you do not use this format if you have limited knowledge of downloading a document.



• Save report for later access - If you check this option, this report will be saved to the Report Manger in My Account in PDF format and will stay there for seven calendar days. You can access this report anytime within the seven-day limit by clicking the View Report icon. If you do not check this option, you will need to re-run the report to access the same information.
  Note: If you check this option once, it will be checked by default next time you run a report. You can decide each time if you want to turn this feature on or off.



• Prompt me for these options for each report – Check this option if you want to see the Request Report window every time you run a report. If you uncheck it, next time you will be brought to the Entitlements Report directly. You will not have a chance to change the report options at this point.
  Caution: It is recommended that you leave this checked all the time. Once you uncheck it, when you need to see the Request Report window, you will have to change the Entitlements Report Options by the Preferences tab of My Account.
  
  
  1. From the My Account window click the Preferences tab.
  
  
  
  2. You will be prompted to Verify Password.
  
  
  
  3. Once you enter the password, click the Continue button. You are brought to the Preferences tab.
  
  
  
  4. Click the down arrow for Other User Settings and select Entitlements Report Options from at the bottom of the list.

  5. Check the box by Prompt me for these options for each report.
  
  
  
  6. Click the Save Preferences button. You will see “Your Preferences have been Saved” in the middle of the page.

Report Details

The following is a breakdown of all the sections in the report, including samples of each section. Most of the information presented is self-explanatory.

The details of each section is collapsed by default. Click the icon to close the details of a section; click the icon to expand the details again; click the icon to print the section; click a link (in blue) to go directly to that specific section of the report.

Please pay special attention to Address Summary, Address(es) Found, Possible Properties Owned, and Motor Vehicles Registered. These are assets related information. If information is missing from these sections you may want to perform additional searches to get more details.

Subject Information

This section contains subject Name, DOB, age, SSN, and SSN issuance information.

This section lists subject name variations, other names used, or same name with different DOB information. These are normally inaccurate data fed by the various data sources.

Indicators

This section gives you a snapshot of the subject’s overall financial well-being: if he/she ever filed for bankruptcy; if he/she owns property; or if he/she has a small business.

The example below indicates the subject filed for bankruptcy, owns a property, and has no business connections.

Address Summary

Here is a list of subject’s current and previous addresses within the time frame picked for the report. The one with a check mark is the current address.

Note: If you do not see a check mark, the address with the most recent date may be used as the current address.

Others Associated with Subject SSN

This section lists all the persons that share the subject’s SSN. These are normally inaccurate data (for example typos, spouse’s SSN) from the various data sources.

Here is a summary of all the findings in the report. Clicking a link (in blue) will bring you directly to that specific section of the report.

Bankruptcies

This section lists all the bankruptcies under the subject’s name. Each record has the debtor, creditor, court, attorneys, and case information. The record stays in the system for 10 years. Bankruptcy records 10 years and older may have been purged.

This section lists all the liens and judgments against the subject.

UCC filings are normally associated with businesses.

Here lists subject’s wireless phone number(s).

Here lists the subject’s employment information within the time frame selected for this report.

Note: This database has limited information. It may not show the entire employment history of the subject.

The following is additional information on subject’s current and historical address information. There are three subsections: Address Summary, Active Address(es), and Previous And Non-Verified Address(es).

Address Summary repeats what is captured in the previous Address Summary section.

Active Address(es) is information on the subject’s current address. In addition to Name Associated with Address and Neighborhood Profile, there is Property Ownership Information for this Address.

Previous And Non-Verified Address(es) contains information on the subject’s historical address(es). The data structure is exactly the same as the Active Address(es).

Possible Properties Owned by Subject

This section lists all the properties that are tied to the subject.

Note: If you see multiple property records, to verify the data, you may want to run a separate Property Deeds search (using address) under menu Assets, where all the real estate transactions related to the subject are recorded. The search results list will indicate if the subject is the owner or the seller of a property. If the subject is the seller, except in the case of refinance, the subject no longer owns the property.

Motor Vehicles Registered To Subject

This section also contains both current and historical data. The Record Type under Registrant(s) will indicate if the record is CURRENT or HISTORICAL.

This section lists the subject’s criminal offenses. But not all the records presented are criminal in nature. The example below is a traffic citation.

Here lists the professional license(s) of the subject. You may see multiple records for the same license, but with a different year.

Possible Associates

Lists persons that are related to the subject in different ways. They can be the business associates, people from a shared address, or person with the same/identical names.

Possible Relatives

Lists the spouse, sons/daughters, parents, of the subject. There are two sub-sections: Possible Relative summary, which is just a list of names, and Possible Relatives, which has details on the individuals on the summary list. The number of “>” indicates the degrees of separation. A single > represents a first degree (higher likelihood via shared addresses and names) relative while a >>> indicates a third degree relative (lesser likelihood – shared address of one relative is the same shared address of another relative, thus a possible connection). Occasionally, aliases (AKA) for an individual will be present within this list and hierarchy as well as other true individuals.
